What We Do |
Service CoordinationThrough Service Coordination we help to assist individuals and family members to gain access to necessary services and supports appropriate to an individual's needs and life goals. Assistance is provided by qualified Service Coordinators and uses a person centered process to develop, implement and maintain an Individualized Service Plan (ISP). Frequently families who receive service coordination from Ability Beyond Disability choose to receive other services from us as well. We offer a wide array of services and supports including self-advocacy, transition planning, crisis intervention, housing modifications and travel/leisure planning. The purpose of Service Coordination activities is to afford every individual a greater sense of integration and independence in the community. |
