    NY OMRDD HCBS Waiver Services

    Eligibility: must have a formal diagnosis of mental retardation or other developmental disability which includes: autism, cerebral palsy, epilepsy, mental retardation, and/or other neurological impairments; the onset of this disability must have been manifested prior to age twenty-two; must receive New York Medicaid; must have a Medicaid service coordinator to be eligible.
