    NY DOH HCBS/TBI Waiver

    NY DOH HCBS/TBI Waiver Eligibility (must meet all of the following criteria): A New York Medicaid recipient who is diagnosed with TBI or other type of brain injury; 22-64 years old with the injury occurring on or after 22nd birthday; eligible for a nursing facility level of care; chose to live in the community rather than in a nursing facility; can identify a safe living arrangement that will meet their needs; can be supported safely within the community with the funds and services available under the DOH HCBS/TBI Waiver.

      Home and Community Support Services
      Intensive Behavioral Program
      Environmental Modification
      Respite
      Special Medical Equipment and Supplies
      Transportation
