How to Receive Services
Ability Beyond Disability will tailor its services based on the unique requirements, needs, desires and expectations of each individual. Based on its service philosophy of person-centered planning, each person will undergo an assessment and recommendation prior to service placement.
The Three-Step Process:
- Place an inquiry through the Information and Referral Department to request services and determine available funding. (203) 826-3072 or info@abilitybeyonddisability.org
- An evaluation process will be conducted to determine our ability to provide services.
- Admissions and Service Placement processes will be completed.
Funding sources are varied and assistance can be provided in pursuing the application process. Private-pay is also accepted.
Medicaid Waiver Programs
Waiver Programs, authorized under section 1915(C) of the Social Security Act, provide states with greater flexibility to serve individuals with substantial long-term care needs at home or in the community rather than in an institution. The Federal Government "waives" certain Medicaid rules. This allows a state to select a portion of the population on Medicaid to receive specialized services not available to all Medicaid recipients.
Ability Beyond Disability is an approved provider for three Medicaid Waiver Programs:
- CT TBI Waiver: Connecticut Traumatic Brain Injury Medicaid Waiver
- NY DOH HCBS/TBI Waiver: New York State, Department of Health, Home & Community Based Services Medicaid Waiver for Individuals with Traumatic Brain Injury
- NY OMRDD HCBS Waiver: New York State, Office of Mental Retardation and Developmental Disabilities, Home & Community Based Services Medicaid Waiver
